The equal split divides the total bill (plus tip) evenly across everyone.
The itemized split instead works out each person's share of the tip in proportion to what they actually spent — so someone who ordered a £30 meal pays more of the tip than someone who ordered a £10 side, even though the tip percentage is the same for the whole table.
